Returning Student Registration Form
The Returning Student Registration Form (formerly Demographic Updates Form) is available to families from September 9 to October 5. This form is accessed via the Parent Portal and is an opportunity each year to review and update student information and also includes our yearly permissions and consents.

Lockers

Students who would like a locker at CHDHS should log into your GNSPES and go to this link: https://docs.google.com/forms/d/e/1FAIpQLSf2bceDwAZjSQmyqDN0bwVOowiXHdySj9T1TUsOPoM8XqfzcQ/viewform?usp=header

 
Locker notes:
 
Students are not to use their own locks, but use the locks and combination provided
 
At the end of the school year, All lockers must be emptied even if you are returning the following year
 
Your locker will stay the same for your time as a student at CHDHS
 
The administration can go into your locker if necessary, and all items in your locker must meet our school code of conduct
 
Lockers will be assigned on a first come first serve basis. You will get your locker # and combination emailed to you within a week of completing this form, hopefully sooner!
